Information for Incoming Students and their Families (updated 4/18/19)
~Click here to complete the Bio/Academic Information Form as soon as possible if you haven't submitted. Complete the Bio/Academic Information Form online if you will be attending Bronx Science in the Fall of 2019. The deadline to submit the form was April 7, 2019.

Bio/Academic Form deadline is April 7, 2019. (submit online here) If you haven't submitted already please do so as soon as possible.
Child & Adolescent Health Examination Form - Students bring to orientation on June 6th, or can be mailed to the school. Form can be found here.
Sports Physical Exam Form - Complete and KEEP UNTIL student attends “try-outs” for sports teams.
Asthma Action Plan - If student has Asthma bring completed form to orientation on June 6th or can be mailed to the school.
Health Education Parent Opt-Out Form - Read, decide, return to Bronx Science on June 6th or mail by June 17th, in a sealed envelope to the school.
Blue Data Form - Complete and bring to orientation on June 6th, or can be mailed to the school.
Residency Questionnaire - Complete and bring to orientation on June 6th, or can be mailed to the school.
IEP/504 Form - If student has an IEP/504, mail to the school by May 5, 2019, attention Sandra Smith.
Register for Music Auditions (if interested) Audition information and registration form is here. Register for an audition by May 28th.
Bio/Academic Form submit online by April 7th. (If you haven't submitted already, please do so as soon as possible.)
Summer School Registration (optional) - to May 28th at 3:00 PM.
Student Orientation - June 6th (bring completed forms).
All Outstanding Forms due by mail by June 25, 2019.
